A cell therapy first developed against cancer has markedly pushed back severe, treatment-resistant rheumatoid arthritis in a first-of-its-kind clinical trial. A team at Charité – Universitätsmedizin Berlin treated six patients with a CD19-directed CAR-T cell therapy. Disease activity fell in all of them, and three patients reached sustained, medication-free remission. The results appeared in late August 2026 in the journal Nature Medicine.
Rebooting a misfiring immune system
In CAR-T cell therapy, a patient’s own immune cells are engineered in the lab to recognise and destroy the B cells that drive disease. In the trial, known as COMPARE, the six patients – three women and three men aged 31 to 69 – received a single infusion of the product mivocabtagene autoleucel after conventional antirheumatic drugs had failed.
After treatment, autoantibody levels dropped sharply and inflammatory foci in the joints were no longer detectable. Lead investigators David Simon and Gerhard Krönke describe the effect as a targeted “reset” of the part of the immune system that fuels the inflammation.
Small sample, early stage
Despite the encouraging data, the researchers urge caution. This is the world’s first trial of its kind, involving just six patients. Responses varied, and in one person the disease returned after an initial remission. At this early stage, no claim of a cure can be made. A larger comparative study with ten more patients is planned.
